Terms and conditions for Teacher Pass 2024

This contract is made between the client and FIT4MOM South Bay and is valid for teachers, school administrators and staff for unlimited classes over the summer and teacher holidays for the rest of 2022 (mental health days included). Unused classes expire on January 8, 2023. Unused classes DO NOT roll over to 2023. Teacher class pass is non-refundable and non-transferrable.

Teacher pass includes unlimited classes during summer break. Plus, the following 2022 holidays:
Labor Day
Staff Development Days
School observed holidays
Veterans Day
Thanksgiving week
Winter break (ending 1/8/2023)
Mental health days

I authorize FIT4MOM South Bay (ASC Fitness & Nutrition) to initiate a charge to the account entered into Pike 13 for my classes.
No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the South Bay franchise.
